Herbal supplements may raise blood lead levels
December 2nd, 2009Some herbal supplements may boost the levels of lead in the blood of women, new research shows.
Among 12,807 men and women age 20 and older, Dr. Catherine Buettner, at Beth Israel Deaconess Medical Center in Boston, Massachusetts, and colleagues found blood lead levels about 10 percent higher in women, but not men, who used specific herbal supplements.
When they examined herbal supplement use among women of reproductive age (age 16 to 45 years old), “the relationship with lead levels was even stronger, with lead levels 20 percent higher overall, and up to 40 percent higher among users of select herbal supplements compared to non-users,” they report in the Journal of General Internal Medicine.
Lead accumulates in the body over time and may pass from a woman’s placenta and breast milk to developing fetuses and infants. The U.S. Food and Drug Administration does not specify safe lead limits, or even routinely test for this toxin in herbal supplements.
Buettner’s team found that women using Ayurvedic or traditional Chinese medicine herbs had lead levels 24 percent higher than non-users, while those using St. John’s wort and “other” herbs had lead levels 23 percent and 21 percent higher, respectively, than non-users.
When combined with prior studies hinting at excess lead in specific supplements, the evidence strongly suggests use of specific herbal supplements may result in higher lead levels among women, Buettner said.
In the current study, Buettner was reassured to find “no evidence of lead toxicity,” she told Reuters Health in an email.
The researchers point out that the use of some herbal supplements among study participants was low, which limited the power to detect associations among specific herbal supplements.
They also emphasize that the current study does not prove that herbal supplements cause higher lead levels. They urge further studies to analyze how other lead exposures, calcium intake, or use of other dietary supplements alter lead levels.
Dr. Adriane Fugh-Berman, at Georgetown University Medical Center in Washington, D.C. concurs in an editorial on the study, and also cautions, “let us not use too broad a brush to tar all herbal products.”
Specific analyses of specific herbal products or the blood of users, Fugh-Berman writes, should be used to establish products containing problematic amounts of lead.
Timely and continuous HIV care extends survival
December 2nd, 2009In people infected with human immunodeficiency virus (HIV), the virus that causes AIDS, high-risk behavior, HIV infection itself, as well as late initiation and early discontinuation of anti-HIV therapy all contribute to substantial decreases in life expectancy, United States researchers report.
Using a comprehensive computer model of HIV disease, the researchers simulated cohorts of HIV-infected individuals and compared them with uninfected individuals who had similar demographic characteristics.
“We estimated that for people in the United States living without HIV, life expectancy beginning at age 33 is an additional 43 years,” first author Dr. Elena Losina, of Massachusetts General Hospital, Boston, told Reuters Health.
“High risk behavior, such as substance abuse, on average, accounts for an 8 year reduction in life expectancy,” Losina added.
HIV infection itself reduces life expectancy by an additional 12 years and a late start or premature discontinuation of HIV treatment further reduces life expectancy to a total of 23 years, she and colleagues report in the journal Clinical Infectious Diseases.
“The current therapy for HIV is very effective,” Losina commented, “but unless HIV-infected patients initiate treatment on time and stay on treatment, the treatment benefits would be truncated substantially.”
Minorities, especially minority women, tend to initiate therapy later and are more likely to drop off from care, and this “leads to disproportional losses in life expectancy in these population groups,” she said.
“There is a critical need for people with HIV to begin care in a timely way and to remain in care,” senior investigator Dr. Kenneth A. Freedberg, also from the Massachusetts General Hospital, said in a statement. “HIV testing for all adults in the United States, as currently recommended, with effective linkage to care, will have important survival benefits.”
Many pregnant women take drugs harmful to baby

In a study, they found that many pregnant women still take medications long known to cause birth defects.
Some medications with known fetal risk, such as drugs that control epilepsy, are essential during pregnancy, Dr. Anick Berard, at the University of Montreal in Quebec, noted in an email correspondence to Reuters Health.
Other medications, such as those that treat severe acne, anxiety and psychiatric drugs, antibiotics, and many drugs prescribed for heart disease and medical conditions, “can and should be avoided,” according to Berard.
Women should understand the side effects of any drug they are taking — especially drugs treating a chronic condition — and plan pregnancies to avoid or minimize risks such drugs pose to babies, Berard added.
For the 5 years between January 1998 and the last day of 2002, Berard and colleagues analyzed the prescriptions filled by pregnant women for drugs available at the time and known to pose fetal risks.
Their report, in BJOG: An International Journal of Obstetrics and Gynecology, shows 56 percent of 109,344 pregnant women filled at least one medication prescription. A total of 6.3 percent (6,871 women) did so for at least one medication known to pose a risk to the fetus.
“These pregnancies were associated with an elevated number of (pregnancy terminations) and babies born with major (birth defects) in comparison with the expected numbers in the population,” they note.
Specifically, terminations occurred in 47 percent of the pregnancies exposed to drugs with known fetal risks. Six percent of these pregnancies ended in miscarriage.
By contrast, in the much larger non-exposed group about 36 percent of the pregnancies had been terminated and fewer than 5 percent ended in miscarriage.
Berard’s team further identified birth defects in 8.2 percent of 2,842 infants exposed to risky drugs during gestation and available for assessment, compared with 7.1 percent of the 59,287 infants not exposed. This is “a statistically significant difference,” they note.
They emphasize, however, that it cannot be concluded that the drug exposure caused the birth defects. These pregnancies may have also been exposed to other harmful agents or maternal health conditions, they point out.
The investigators call on doctors caring for women of childbearing age to conduct a thorough medication review prior to a planned pregnancy, or as soon as an unplanned pregnancy is recognized.

More than 1,000 deaths in past week from H1N1: WHO
November 29th, 2009More than 1,000 deaths from the H1N1 swine flu virus were officially reported in the past week, a sharp rise which brings the global total to at least 7,826, the World Health Organization (WHO) said on Friday.
More than half of the latest fatalities were reported by health authorities in the Americas region.
The winter flu season arrived early in the northern hemisphere this year and continues to be intense across parts of North America and much of Europe.
“In the United States and Canada, influenza transmission remains very active and geographically widespread,” the WHO said, adding that the disease now appeared to have peaked in all U.S. regions.
“In Canada, influenza activity remains similar but (the) number of Hospitalizations and deaths is increasing,” it said.
It is too early to say whether there has yet been a peak in infections in the northern hemisphere, the WHO’s top flu expert said on Thursday, and it will be some weeks before there is a downward trend in the numbers of those catching the virus.
The H1N1 pandemic virus is causing widespread and increasing infections in Europe, with many reporting a rapid rise.
Sweden, Norway, Moldova and Italy are reporting “very high activity” and health care services are reeling under the strain in Albania and Moldova, it said. Flu has peaked in other European countries including Belgium, Ireland and Serbia.
Flu transmission is active in East Asia and it remains “stably elevated” in Japan, but may be decreasing slightly in cities there, according to the United Nations health agency.
In temperate zones of the southern hemisphere, little pandemic flu activity has been reported.
Does cataract surgery make eye disease worse?
November 29th, 2009Cataract surgery does not appear to worsen the course of age-related macular degeneration (AMD), a progressive loss of vision due to deterioration of nerve tissue in the retina, a new study shows.
The finding, however, does not yet settle the long-standing controversy over the appropriateness of cataract surgery in eyes with this disorder, an editorial warns.
AMD is the most common cause of blindness in adults 55 years of age and older.
In the latest issue of the Archives of Ophthalmology, Dr. Neil M. Bressler from Johns Hopkins University School of Medicine, Baltimore, and colleagues describe the outcomes of cataract surgery performed on 86 eyes with AMD from 86 elderly patients.
One week after surgery, imaging showed new AMD lesions in 5 eyes, but the authors believe these lesions were present before surgery; they just weren’t visible.
By month 12, new lesions had been diagnosed in 3 additional eyes. After excluding deaths, eyes with missing or poor images, and the 5 eyes that probably had AMD before surgery, the AMD progression rate from week 1 to month 12 was less than 5 percent (3 of 65 eyes).
This rate was not significantly different from the rate in the subjects’ unoperated eyes; only one (3.3 percent) developed new AMD lesions during the same period.
Dr. Barbara E. K. Klein from the University of Wisconsin, the author of the editorial, points out that several studies since the late 1980s have indicated an association between cataract surgery and AMD progression.
In light of inconsistencies in these studies, Klein advises, a “straightforward discussion” between the patient and his or her doctor regarding risks of progression of early AMD and development of late AMD and its accompanying risk of vision loss after cataract surgery.
Having this discussion will help “both the patient and his or her physician to make a more informed decision,” Klein wrote.
